Investigation Summary
At approximately 10:33 a.m., on July 11, 1984, Employee #1 was assisting another employee using a core drilling machine. They were drilling four inch diameter holes in a concrete floor. The holes were twenty-feet-two-inches above another concrete floor. The floor edge was guarded only by a one inch manila rope fastened to one-and-three-quarter-inch black pipe. The rope was twenty-feet-forty-two-inches high and very slack. The core drilling machine jammed throwing Employee #1 off the concrete floor he was standing on. He landed on the concrete floor twenty-feet-two-inches below. Employee #1 was killed instantly due a skull fracture and internal injuries.
