Investigation Summary
On April 14th or 15th, 2020, Employee #1, a maintenance mechanic, began to experience symptoms associated with COVID-19. He tested positive for COVID-19 on April 17, 2020. On April 20, 2020 at approximately 3:30p.m. to 4:00 p.m., Employee #1 started to experience shortness of breath and went to the hospital. He died shortly after. It is possible that Employee #1 suffered from pre-existing respiratory conditions.
