FINGERS AMPUTATED BY POWER PRESS

ON AUGUST 8, 1984, AT 1:18 PM, EMPLOYEE #1 WAS CHECKING AND REMOVING A STUCK PART IN THE UPPER DIE OF A FEDERAL PORT REVOLUTION MECHANICAL POWER PRESS, SERIAL NUMBER 10-359, COMPANY NUMBER 31. HE WAS TRYING TO DETERMINE WHY THE PARTS WERE STICKING. HE WAS REACHING INTO THE POINT OF OPERATION, REMOVING THE PART, AND HAVING THE OPERATOR TRIP THE PRESS. THIS PROCEDURE WAS REPEATED SEVERAL TIMES. THE LAST TIME, AS HE HAD HIS FINGERS IN THE DIE TO REMOVE THE STUCK PART, THE PRESS TRIPPED AND THE DIE CLOSED ON HIS MIDDLE AND INDEX FINGERS OF HIS LEFT HAND, COMPLETELY AMPUTATING THEM. HE WAS HOSPITALIZED AS A RESULT OF THE ACCIDENT.
