Investigation Summary
At approximately 2:30 p.m. on April 16, 2020, an employee was working full time on the night shift. Her last shift worked was March 29, 2020. She then took five shifts off due to the bereavement of a family member. She called on April 7, 2020 to report that she had a fever and was going to be tested. On April 9, 2020, the supervisor, received notification in writing that the employee was under quarantine and would be unable to return to work until cleared after retesting. The employee died from COVID-19 infection on April 16, 2020.
