Employee with COVID-19 infection dies

On March 12, 2020, March 20, 2020 and March 24, 2020, Employee # 1, a Registered Nurse, and a coworker were on the same shift. Both were asymptomatic and passed employer screening. Both were also working at other sites not related to the employer in question. On March 25, the coworker went to the hospital ill and was quarantined at home. Employee # 1 took leave without indicating a reason on March 27, 2020, his last day at work. On March 28, 2020, the spouse of Employee # 1 notified the employer that Employee # 1 was hospitalized. On April 3, 2020, COVID-19 results for both employees were reported as positive. On April 14, 2020, Employee # 1 died. The coworker was still at home recovering and was not hospitalized. The employer learned that Employee # 1 was hospitalized on March 28, 2020, and that his test results were positive on April 3, 2020. The employer claimed that they were notified that Employee # 1 died on April 14, 2020. They saw the notice in the obituaries and reported to OSHA on April 15, 2020.
