Investigation Summary
At 9:15 a.m. on January 30, 2020, an employee was operating a stand-up PIT (Hyster Model: E40HSD, Serial Number: A219N02875E). It was standing idle in the isle at the west end of the warehouse, when a sit-down PIT (Clark Model: GPS 1S, Serial Number: GP127-9049-9240K0F), operated by a coworker, collided with the Hyster. The left fork of the Clark PIT entered the stand-up cab of the Hyster PIT and struck the right foot of the operator. The Hyster PIT operator received an injury of the right foot, requiring surgical amputation of the right big toe at the base of the toe. The employee was hospitalized.
