PRECAST CONCRETE PANEL FELL ON EMPLOYEES.

ON APRIL 11, 1984, AT 3:30 - 3:45 PM, EMPLOYEES #1,2, AND 3 WERE PLACING CRIBBING FOR SUPPORT OF AN OUTRIGGER ON A GROVE CRANE, SERIAL NUMBER 33674, MODEL RT 655, WHEN THEY WERE STRUCK BY A FALLING PRECAST CONCRETE PANEL WEIGHING 21,372 POUNDS. THE PANEL WAS BEING SET ON THE EAST WALL OF THE BUILDING , BUT HAD TEMPORARILY BEEN PLACED ON THE GROUND, AND SUPPORTED AGAINST THE BUILDING STEEL WHILE THE CRIBBING WAS BEING COMPLETED. EMPLOYEE #1 HAD BEEN PICKING UP CRIBBING MATERIAL AND WAS NEAR THE CENTER OF THE PANEL WHEN IT FELL ON HIM, CAUSING FATAL HEAD INJURIES. EMPLOYEE #2 WAS ON THE NORTH SIDE OF THE CRIBBING OPERATION AND WAS PUSHED 10 - 15 FEET TO THE NORTHEAST, SUFFERING ABRASIONS ON HIS RIGHT ARM. EMPLOYEE #3 WAS ON THE SOUTH SIDE OF THE CRIBBING OPERATION AND WAS PINNED UNDER THE PANEL. HE SUFFERED A BROKEN CLAVICLE AND A NECK LACERATION. THE PANEL HAD BEEN LEFT STANDING UPRIGHT , UNSECURED, ON SOFT SOIL WHILE THE EMPLOYEE'S ATTENTION WAS FOCUSED ON THE CRIBBING OPERATION. THE CRANE OUTRIGGER HAD NOT BEEN SUPPORTED PROPERLY FOR THE EXISTING SOIL CONDITIONS.
