Investigation Summary
At 1:50 p.m. on February 19, 2020, an employee working as a production helper for a food manufacturer was purging excess product from a machine. The employee then removed a pipe in order to clean inside a ball valve before instructing a coworker to shut off the product supply button. The employee proceeded to swipe his right index finger inside the valve when the valve closed on his finger. The employee amputated his finger and required medical treatment without hospitalization.
