Investigation Summary
At 9:30 a.m. on January 14, 2020, Employee #1 and a coworker were removing a heavy metal room divider from the ceiling. They misjudged the weight of divider, and when it tilted, they could not stop it from falling over and crushing the leg of the employee. The employee was hospitalized for a broken leg. The employees failed to recognize the hazard of releasing the divider and underestimating the weight of the divider.
