Investigation Summary
At 11:30 a.m. on February 11, 2020, an employee working as a sales supervisor for an armored car service was troubleshooting a conveyer. The conveyor at the coin packing line was moving too slow and bouncing. The employee caught his left hand in the nip point between the belt and roller pulling off the skin, nail and fingertips. The employee amputated his index and middle fingertips and required medical treatment without hospitalization.
