Investigation Summary
At 11:00 a.m. on January 23, 2020, and employee was operating a Schnell Centro Multifunzione Barwiser 22N Autobender pipe bending machine manufacturing T-25 Standee Rebar. The bend required the employee pull the bar slightly to create a gap needed to make the bend. The employee pulled the bar to esure it made the required square shape that allowed it to stand up. On the 3rd bend, the rotating wheel and rebar struck the inside of the employee's right hand pinning it against the center of the bending wheel. The employee broke his ring and pinky fingers and crushed the middle fingertip. The employee was hospitalized and his pinky finger required amputation.
