Employee's finger is amputated in machine

At 10:15 a.m. on January 13, 2020, an employee was operating a TG7-910 clip jig machine, inserting clips into door parts. The employee was checking clips to ensure they were flush with the clip guide on the TG7-910 clip jig. The employee noticed a clip was not seated flush in the part, and used her right pointer finger to flatten the clip. While she still had her finger on the part, she hit the lever with her other hand, causing the clip insert fork to come down onto her fingertip. The employee's right pointer finger was amputated to the first knuckle and was treated without requiring hospitalization. The machine had a light curtain, however the placement of the curtain was not located in a manner on the machine that would have prevented the employee from being able to reach under the curtain and access the danger zone. No other methods of machine guarding were in use on the machine at the time of the incident.
