Investigation Summary
At 1:30 p.m. on January 9, 2020, an employee working in warehouse of a retail store was operating a Gillis 5500LB Hydraulic Pallet Jack returning an empty pallet to its storage stack. The employee tripped on the pallet jack, lost her balance and fell backwards onto the floor. The employee struck her head on the concrete floor and the pallet fell on top of her. She incurred a head laceration and a concussion, requiring hospitalization.
