Investigation Summary
At 11:25 a.m. on December 5, 2019, Employee #1 was assisting a coworker, a die setting employee, unjam a part from a mechanical punch press. The coworker adjusted the pneumatic air pressure to the press, as Employee #1 was cleaning out debris. A ring on the press moved and crushed the second and third fingers on Employee #1's right hand, resulting in amputation. The employee was treated without hospitalization.
