Investigation Summary
At 3:00 p.m. on December 11, 2019, Employee #1, along with the operator, were removing a piece of blue plastic tarp that was caught in the Blueberry screener machine. Employee #1 reached into the machine. The operator of the Blueberry screener machine either started the machine, or it was already running to assist the tarp removal. Lockout/tagout was not utilized. The left hand (gloved) of Employee #1 became caught in the idler roller of the Blueberry screener machine. Employee #1 sustained fractures and contusions to the left hand/wrist and pinky finger, resulting in hospitalization. Injuries included two broken bones in the wrist and one broken bone in the pinky finger.
