Investigation Summary
At 9:00 p.m. on December 12, 2019, an employee was doing a checklist with a coworker. The employee heard a noise and got onto the empty flowback tank to investigate. The employee told the coworker to close the valve on the other tank. The tank the employee was on exploded, propelling him approximately 30 feet to the ground. The employee was hospitalized to treat the following: lacerated spleen, assorted contusions and lacerations, and a broken pelvis.
