Investigation Summary
At 2:15 a.m. on November 28, 2019, an employee was feeding paper into feed rollers of a napkin machine. While jogging the machine with his left hand (jog button), he attempted to feed the paper into the feed rollers with his right hand. His right fingers (middle and ring) were then caught between the feed rollers. The employee was hospitalized to treat crush injuries to his right middle and ring fingers.
