Investigation Summary
At 12:00 noon on November 1, 2019, an employee inspected bituminous material on a city street and walked back toward her vehicle parked in a church lot. The employee became distracted while walking and tripped on cut tall grass stems and fell onto the paved portion of the church parking lot.
