Investigation Summary
At 4:15 p.m. on October 30, 2019, an employee was operating a Crown RC stand up forklift and bent over to retrieve a pen he dropped. The employee lost control of the forklift and was caught between it and a building support post, crushing his chest and abdomen and was killed.
