Investigation Summary
Employee #1 and a coworker were installing nailers around a roof skylight. Employee #1 started walking toward the northeastern corner of the opposite side of the roof, where materials were standing. The coworker was on his knees at the western end of the bay #4 skylight. Employee #1 turned to the coworker while he was walking and informed him that he was going after more lumber. As soon as he turned around he stepped into an uncovered gravity vent roof opening and fell 46 ft to a concrete floor. He sustained multiple internal and head injuries and died en route to the hospital.
