Employee incurs hematoma in forklift collision

At 9:15 p.m. on October 6, 2019, an employee and a coworker were unloading a semitrailer and placing the product in a warehouse. The coworker was operating a Linde forklift, Model: H32CT, Serial Number: A114145H000T. The coworker exited the semitrailer on the forklift with two 27-inch-high stacked crates on the forks. The employee was operating a Yale forklift, Model: GLC070VXNDSE088, Serial Number: C910V06538R, on an aisle perpendicular to the aisle where his coworker was traveling. He was not wearing a seatbelt. The forks on the employee's forklift contacted the stacked crates on the forklift his coworker was operating, and he flew out of the forklift. The employee told his coworker hat he was only a little sore but otherwise okay and continued to work. After his shift, he was experiencing headaches and went to the emergency room. The employee was hospitalized for treatment of a hematoma.
