Investigation Summary
At 3:19 a.m. on August 29, 2019, Employee #1, employed by a plumbing company, was working at an amusement park. He was in a shored trench and installing a 40-foot long, 18-inch diameter carbon steel pipe when one end of the pipe struck a steel shore. The shore dislodged and fell onto Employee #1. The employee was killed.
