ELECTRIC SHOCK - CONTACT WITH OVERHEAD LINE THRU AERIAL LIFT

AN EMPLOYEE WAS WORKING FROM AN INSULATED AERIAL LIFT OVER THREE PHASES OF A 7200-VOLT OVERHEAD POWER LINE. HE WAS WORKING ON ANOTHER DEENERGIZED, PARTIALLY GROUNDED LINE. HE HAD LOWERED THE AERIAL LIFT BETWEEN ONE OF THE OUTER LINES AND THE MIDDLE LINE. AN UNINSULATED PORTION OF THE UPPER PART OF THE BOOM CONTACTED THE MIDDLE PHASE CONDUCTOR. A CONDUCTIVE PATH WAS SET UP FROM THE CONDUCTOR TO THE BOOM, TO THE METAL-CLAD HYDRAULIC HOSES, TO THE HOSE CONNECTORS, AND TO THE WIRE USED TO HOLD BROKEN TOOL CONNECTORS TO THE SAFETY-BELT TIE-OFF RING INSIDE THE BUCKET OF THE AERIAL LIFT. ANOTHER WIRE, HOLDING A PAIR OF LARGE LINEMAN'S PLIERS (XPJ 53 NICOMPRESS TOOL), WAS TIED TO THE BUCKET AT THIS POINT. THE EMPLOYEE'S KNEE CONTACTED THE LINEMAN'S PLIERS WHILE HE WAS HOLDING THE GROUNDED CONDUCTOR, AND HE WAS ELECTROCUTED.
