Investigation Summary
At approximately 8:30 a.m. on August 15, 2019, an employee was cutting inserts for door sidelights. The employee was using the rip fence and pushrod to cut inserts that were 12 inches in length and 1 3/4 inches in width on a table saw with an unguarded circular blade. The employee's left hand came in contact with the blade while he was performing rip cuts. The employee amputated left middle and index finger and was hospitalized for these injuries.
