Investigation Summary
At 12:00 a.m. on August 7, 2019, Employee #1 was attempting to feed material into a chop saw. Employee #1 was pushing the material into the chop saw, which was not sliding into the jib. The material freed up, and slid easily, and her momentum carried her hand into the chop saw. The guard of the chop saw was not low enough to prevent contact while feeding material. The chop saw blade constantly runs, and after the material is fed, it is lowered with a two-hand control. Employee #1 was treated for finger amputations but not hospitalized. The company performed an accident investigation and added to the barrier guard that was in place to lower it.
