Investigation Summary
At 9:45 p.m. on July 25, 2019, a quality assurance employee was performing a pre-operation visual inspection of in-service (running) equipment located in the Preform-A area of the cookie department to ensure the line was clean of all residual food product. During the visual inspection, the employee noticed a small amount of dough left in the Drop Machine. The employee reached into the running Drop Machine to wipe off the excess product and her right hand got caught between two unguarded rollers. A coworker within close proximity immediately activated the Drop Machine's emergency stop button. Another coworker (maintenance worker) was able to free the employee's right hand from the equipment by reversing the polarity of the Drop Machine's motor. The employee sustained crushing injuries to the right hand. The employee was transported offsite by local emergency medical services and hospitalized due to the extent of fracture and laceration injuries sustained to her right hand.
