Investigation Summary
At approximately 9:00 a.m. on July 20, 2019, an employee was working overtime outside of the normal work duty and shift to perform identified plant maintenance tasks. The employee was a line production worker that typically stacks blocks onto wood pallets. The employee with sealing the forklift shop roof with a product similar to paint using rollers and extension handles and was elevated approximately 12 feet onto the roof using a forklift and a platform. The employee reported the platform was a wood pallet that slid onto the forks of the forklift and the employer reported the platform was a homemade metal platform. The employee sustained serious fractures to his ankle after the employee fell from 12-foot elevation onto the concrete pavement. The employee was hospitalized for these injuries.
