Investigation Summary
At approximately 7:00 p.m. on July 17, 2019, an employee was observing the gluing operation on the lamination machine with the guard raised. The employee reached into the roller feed area to pick a piece of material off as the machine was running and got caught between the two rollers. The employee's right hand was pulled into the rollers and continued to pull him up to his shoulder before the machine was turned off by a coworker. The employee's right-hand index fingertip amputation and sustained two broken fingers. The employee was hospitalized for these injuries.
