Investigation Summary
At 1:30 p.m. on July 14, 2019, Employee #1 and a coworker were constructing a 40 by 18 foot wooden canopy that abutted the large building in the southeast corner of the property. The employee was cleaning up materials and debris when the coworker went to the bathroom and heard a loud crashing sound. A section of the canopy's roof collapsed and struck Employee #1, pinning him to ground. The coworker found him motionless and face-down underneath the wooden structure. Employee #1 sustained a fractured skull and was killed.
