Investigation Summary
At 3:00 p.m. on June 21, 2019, Employee #1 was dispatched by his maintenance supervisor to perform service on a downed machine. The DF-117S strapper machine in the marination department was used to wrap boxes of chicken that weigh 20 pounds. Employee #1 was adjusting the pendulum on the inside of the strapper machine. He had not disconnected the electrical power to the machine. The machine processed a cycle, and his hand became caught in the machine's moving parts. Employee #1 sustained a fracture and laceration to his right finger and hand and was hospitalized.
