Investigation Summary
At 9:15 a.m. on June 12, 2019, an employee was assigned to run a non-destructive tester (NTD) machine while the usual operator was on vacation. The employee was holding a bushing in place attempting to run a test steel bar into the bushing. The procedure the employee used was not appropriate for the machine he was operating despite receiving training by the operator prior to vacation. The employee caught his finger between the bushing and the machine base as the tube was jogged into position. The employee amputated his left index finger and was hospitalized.
