Investigation Summary
At approximately 6:00 p.m. on June 9, 2019, an employee was wrapping issue occurred at the Blade Backing Roll (adjacent machinery) which caused a wrapping issue at the No. 2 Pull Stack. The coworkers that were already addressing the wrapping issue at the Blade Backing Roll attempted to stop the machines from using the normal controls located on adjacent work stations. At the time, the employee exited the tunnel underneath the No. 2 Pull Stack. The employee's fingers were caught in the nip point of the No. 2 Pull Stack's top and bottom rollers. The machine was running at a normal operational speed based on the paper type. The employee amputated three fingers and was hospitalized for these injuries.
