Investigation Summary
At approximately 4:00 p.m. on May 13, 2019, an employee was sliding aluminum into the manual holding device as the operator down the line started the machine to straighten the aluminum. The employee reached over the small tail stretcher placing his hand on the metal to keep it from twisting. His opposite slipped off the handle causing his other hand to move toward the locking mechanism. The employee amputated their fingertip and was hospitalized for their injuries.
