Investigation Summary
At 8:00 a.m. on April 19, 2019, an employee was operating a wire brush machine. The employee was setting up the Borghi U-Twist machine. The machine produced wire brushes to the specified requirements. At the time of the incident, the employee had set the machine to manual mode, engaging the wired remote pendant. The pendant permitted various movement when the correct command functions were selected, while the interlocked doors were open. The pendant required constant pressure on the button to continue the movement. The employee had opened the hinged doors near the carriage. The pendant was placed on the shelf inside the door. In order to determine the correct number of rotations to set the correct number of plates, the employee had to catch and weight the filament that came off the carriage chain. The employee had his right hand in the machine and was operating the toggle control with his left. The carriage moved, pinching his right hand against the wire plates. The employee suffered a severe laceration to the thumb and palm and an amputation to the index finger as a result.
