Investigation Summary
At 2:11 p.m. on May 3, 2019, Employee #1 was assisting his employer, the owner of a roofing company, during renovation work at a two-story single-family residence. The work involved climbing a scaffold constructed by another contractor. The scaffold had no fixed ladder, lacked end rails, and was not fully planked on the first level. As Employee #1 attempted to climb onto the scaffold without using fall protection, he fell, a fall height of 10 feet, and struck the base of a palm tree decorated with rocks. The employee was transported to the hospital, where he was admitted for treatment of a laceration and contusion of the forehead.
