Investigation Summary
At 5:00 p.m. on May 1, 2019, an employee was working with his supervisor troubleshooting a dryer that would not fire up. They were attempting to relight the burner and were getting an error code. The dryer was composed of a rotating drum with a fire box on the end fueled by natural gas. The supervisor was at the control panel (Make: Honeywell, Model: Toshiba) and the employee was approximately 4 feet up on a 10 foot A-frame ladder observing the flame box. When attempting to relight the burner, a malfunction occurred that caused a flash. The flames came out through the back of the flame box and contacted the employee on the ladder. The employee suffered second degree burns to his arms that required hospitalization. The photoelectric eye controlling the lighting/purging sequence was found to have been shorted causing the malfunction.
