Investigation Summary
At 10:30 a.m. on April 4, 2019, an employee was working for a firm that manufactured frozen fruit, juice, and vegetable products. The firm made and sold smoothies and other products. The employee was working as a maintenance technician. He was repairing a citrus machine. He turned it on so that he could spray lubricant on its gears. He was applying lubricant to a moving chain and sprocket. He was using a towel to catch the drips of the lubricant. The towel was tucked into his pants. When he energized the machine, the towel was caught in the gears or by the chain; accounts varied. When he reached in to pull the towel out, the index finger on his right hand was pulled into the sprocket. His right index finger was crushed in the gears. He suffered the amputation of his right index finger. The location was either between the knuckles of the proximal phalanx bone or up to the first knuckle from the fingertip (the distal phalanx). Again, accounts varied. The employee was hospitalized.
