Investigation Summary
At 3:15 p.m. on April 22, 2019, an employee was changing belts on a motor that drives the chains for moving logs. While changing the belts on a motor, the machine was not a lockout, allowing a log to be fed into the electronic eye area. This engaged the section that the employee was working resulting in the right-hand pinky finger getting caught in the belts. The employee suffered an amputation of the right pinky finger just above the second knuckle and was hospitalized.
