Investigation Summary
At 2:30 p.m. on April 16, 2019, Employee #1, employed by a candy manufacturer, was placing a plastic bag over the motor of a mixer. The equipment was running and Employee # 1 performing the cleaning cycle. His fingers were caught in the rotating belts and pulley. Employee #1 received amputation injuries to the fingers on his right hand including but not limited to his index, middle, ring, and pinky fingers. He was hospitalized.
