Investigation Summary
At 4:20 p.m. on March 23, 2019, Employee #1 was adding an extension attachment to a forklift for an aluminum extrusions manufacturer. His head and hand were struck while installing a 55 in long by 3 ? to 4 in wide extension to a Nissan Forklift (Model Number CF50LP). Employee was hospitalized for a laceration on the back of head and crushed right index finger.
