Investigation Summary
At approximately 7:15 a.m. on April 8, 2019, an employee was cleaning windows while standing on a canopy structure at the entrance of the warehouse. The employee was assigned to clean four windows on the exterior of the building, above the metal and partially glass canopy. He accessed the canopy by climbing a 20 feet section ladder. The employee was using a handheld sponge and squeegee on extension poles to clean the windows. While cleaning the windows, The employee stepped on the canopy screen and fell through the screen, falling 15 feet from the top of the canopy directly onto the ground below. The employee suffered blunt head trauma, concussion, an 8cm stellate frontal scalp laceration, nasal fracture, open comminuted intra-articular olecranaon fracture, bilateral knee abrasion, and transverse fracture of the right patella. The employee was hospitalized for these injuries
