Investigation Summary
At around 6:15 a.m. on April 2, 2019, Employee # 1 was on a ladder, reaching for a box to be sent out. He was assigned by his supervisor to pull out parts from the racks for the morning delivery. Employee # 1 was on the top rung of a ladder and wanted to get an item from the top shelf, which was approximately five to six feet above his head. He reached up and grabbed the racking to pull himself higher. He lost his balance and fell down on the steps of the ladder and onto the concrete floor. He suffered multiple body fractures and was hospitalized for six days. The employer is an auto parts whole seller. DOSH was notified by the employer the next day at 2:36 p.m. The employer satisfied the reporting requirement. DOSH arrived at the site on April 8, 2019. The accident could have been prevented if the employer had implemented an adequate system designed in accordance with Section 3276 (e)(A)(15). The employer was cited with a serious accident for a violation of Section 3276 (e)(A)(15) for failure to protect the employee from the hazard.
