Investigation Summary
At 10:15 a.m. on April 4, 2019, Employee #1 and coworkers, employed by a steel erection company, were working at the construction site for a commercial building. Employee #1 was walking and guiding a load of construction material being moved by a coworker operating a Telehandler, JLG THO816-1 G9-43A, when the telehandler/forklift ran over his right foot. Employee #1 sustained crushing injuries to the foot. He was transported to the hospital, where he was admitted for treatment, including surgery to the foot.
