Investigation Summary
At 3:00 p.m. on March 22, 2019, an employee was sitting in his office. Tube Pusher #3 malfunctioned and the Fin/ Tube Coordinator called over the intercom for the Scrap Team to come to take a look at the machine. The employee proceeded to Tube Pusher #3 to see what the problem was. As the employee was troubleshooting the problem, he placed his head near the hammer slide on Tube Pusher #3. Noticing the hammer was stuck on the end of the slide, he was attempting to see what caused the issue. The light curtain on the machine was inoperable at this time due to the PLC programming only called for the light curtain to be working when the hammer was in a forward movement. The hammer had completed it's forward movement and was attempting to return to the home position. The employee was struck above his left eye by the hammer when it broke free and returned to the home position. The employee suffered a laceration above the left eye and a fractured skull. The employee was hospitalized.
