Investigation Summary
At 3:00 a.m. on April 5, 2019, an employee was pushing a cylindrical tube steel barrel over to the opposite side of a straightener steady rest when a hydraulic jack that was being used as a fulcrum fell over, catching his left hand index finger between the barrel end and a metal steady rest. The employee amputated his finger and was hospitalized.
