Investigation Summary
At 3:45 p.m. on April 5, 2019, an employee was standing on a six foot ladder while lining up an engine that was on mounting brackets as a coworker was sing a forklift. He employee fell from the ladder when the coworker struck it with the forklift they were driving and the employee struck his head on the engine. The employee fractured his skull and sustained serious lacerations as well as abrasions to his head. The employee was hospitalized.
