Investigation Summary
At 8:00 a.m. on March 30, 2019, an employee was working for a firm that manufactured urethane and other foam products. He had started work at 7:00 a.m. He was performing maintenance on an Orion stretch wrapper machine. He was going to replace the drive shaft and shave keyways for the drive gears of a wrapper bar on the machine. He shut off power and locked out and tagged out the machine prior to beginning work. He then set up an A-frame ladder (in a closed position) to access the chain at the top of the machine. The employee took the first outside chain out of the sprocket and began to remove the second chain, a single strand 60-pitch roller chain. When the employee pulled up on the second chain, the chain pulled downward, pinching the fingertips of his middle and ring fingers. When the employee tried to pull his hand away, the tips of his fingertips were sliced and amputated. He then came down from the ladder clutching his hand, and he was taken to the hospital. The employee suffered amputation of the fingertips on his middle and ring fingers. He was hospitalized.
