Investigation Summary
At 3:15 p.m. on March 20, 2019, Employee #1 was operating a tugger to move equipment from Plant 5 to Plant 4. The employee had been trained in safe operation of the tugger prior to operation. It was sprinkling rain and the surface was wet. As the employee was coming around to the overhead door, she lost control of her tugger. In an attempt to brace for impact into the guardrail, the employee stuck her left foot out. Her foot was crushed between the guardrail and the tugger, resulting in fractures to her foot. Employee #1 was hospitalized for treatment.
