Investigation Summary
At 7:00 a.m. on February 20, 2019, Employee #1 was working in the central kitchen area overseeing and helping in the preparation of food. At the time of her incident, she was operating an Automatic Lidding Machine (Model Number HHA6002070) that places lids of food containers. She saw that food containers were not coming out straight when she reached inside the machine to grab the containers. A rotating part of the automatic lidding machine amputated the tip of her index finger. The employee was hospitalized.
