Investigation Summary
At 5:30 a.m. on March 21, 2019, an employee was working on the mezzanine above the packing department of a cheese production facility. The employee stepped from the mezzanine onto a 24 foot Werner Fiberglass Extension Ladder to climb down to the chemical room floor and the ladder slid out from underneath him, causing the employee to fall. The employee struck his chin against a ladder rung and fractured it. The employee was hospitalized.
